Invasion or microinvasion Large loop excision of transformation zone (LLETZ): may need admission for EUA and staging.
Severe dyskaryosis LLETZ at first visit
Moderate dyskaryosis LLETZ at first visit

Postcoital bleeding
Colposcopic assessment and biopsy.
Biopsy negative or CIN I
Repeat colposcopy/biopsy in 6 months.
Biopsy CIN II, III or worse
LLETZ
Borderline changes or severe inflammation Colposcopic assessment and biopsy.
Biopsy negative or CIN I
Repeat colposcopy/biopsy in 6 months.
Biopsy CIN II, III or worse
LLETZ
Pregnancy Colposcopic assessment still appropriate, but unless suspicion of invasive disease, biopsy can be deferred until 6-8 weeks post delivery.

In general, LLETZ performed under local anaesthetic, although certain lesions or certain patients may require a general anaesthetic.
