Blood glucose and myocardial infarction

DIGAMI (Diabetes Mellitus, Insulin Glucose Infusion in Acute Myocardial Infarction)

There is evidence that insulin-glucose infusion followed by a 3/12 subcutaneous insulin regime improves long-term prognosis is known and newly diagnosed patients with diabetes following acute myocardial infarction (J.Am.Coll Cardiology 1995, 26:57:65). The greatest degree of benefit is seen in patients not previously treated with insulin (Eur. H.J. 1996, 17 :1337-1344).

Criteria for insulin infusion for first 48 hours following myocardial infarction: 

Cardiac Criteria

DIGAMI

MI + Diabetes or MI Lab glucose >11mmol/l on admission

Insulin infusion (for 48 hours)

3/12 sub-cut insulin regimen Depending upon individual circumstances. May go back to sulphonylurea.

These patients will receive their care in the first instance from the specialist team.
