The role of the Locality |
The content of the PDP is collated with all others in the area to provide information about global learning needs for the GP tutor and clinical governance lead. This collated information will be anonymised before publication. The form 4 is kept confidentially by the Locality office, and its individual contents not disclosed to outside parties without the express consent of the appraisee. This maintains confidentiality of the outcome of the individual’s appraisal. Form 5 could be used by the appraiser and the appraisee to define or detail any confidential areas not appearing on form 4. Forms 3 and 5 remain the property of the appraisee.
