The appraisers guide to the appraisal process

 

  1. The appraiser receives requests for appraisal from the appraisal administrator.
  2. It is suggested that appraisee uses the NHS appraisal toolkit - online appraisal at this website is recommended for 2005 onwards.
    ( Forms 1 to 3 and   Form 4 (modified) are available to appraisers if appraisees do not wish to use the appraisal website)
  3. Each appraisee will be contacted by their appraiser to set a date for the appraisal.
  4. The appraisee informs the appraiser that the online forms are completed or forwards   Forms 1 to 3 to the appraiser electronically, together with any evidence, to the appraiser at least 2 weeks before the appraisal date. The appraisee should also send his last form 4 and the completed PDP template from his/her previous appraisal.
  5. The appraisal interview takes place and from this the appraiser and appraisee together determine the content of form 4 and subsequently the personal development plan (PDP).
  6. The form 4 and PDP are agreed by the appraisee and appraiser
  7. The PDP is signed off by the appraiser  and then forwarded electronically to the locality GP tutor
  8. The content of the PDP is collated with all others in the area to provide information about global learning needs for the GP tutor and clinical governance lead. This collated information will be anonymised before publication. The form 4 is kept confidentially by the locality, and its individual contents not disclosed to outside parties. This maintains confidentiality of the outcome of the individual’s appraisal. Form 5 could be used by the appraiser and the appraisee to define or detail any confidential areas not appearing on form 4. Forms 3 and 5 remain the property of the appraisee.
  9. Feedback from the appraiser will be requested – “how was it for you?” The completion of the feedback form by the appraisee and the appraiser will then trigger payment: the appraiser will receive a sum of money to compensate for the locum costs required for them to take protected time away from their practice work. The feedback forms will be collated by the appraisal administrator.

  10. An appeal system is available for those appraisers who have an issue about the process of the appraisal which they cannot address with their appraisal partner to their mutual satisfaction. This process will address the process of the appraisal or specific problems resulting from the appraisal, but cannot address the outcome of the appraisal.
  11. The appraisee’s last form 4 and PDP forms the basis for the subsequent year’s appraisal.
