If a request is sent to the district nurse to take blood, a blood form should be completed and put in the district nurses tray. The clinical information on the form sent to the district nurse must include the current problem. |

| Take blood. | ||
| Complete form. Make sure the details include the
patient's date of birth. The clinical details MUST begin with the
current problem heading for the reminder. The name of the
clinician requesting the blood tests should be entered on
the request form unless stated otherwise. Bloods taken according to secondary prevention protocols are assigned to the following nurses on the request form:

Care must be taken that the text for the requesting
clinician is added to the form. This may be a doctor or nurse1, nurse2
etc
