Blood tests

Step Action Notes
Patient sees doctor or nurse. Enter or select current problem. If a blood test is requested, there must always be a current problem because this is an ongoing clinical event.
Enter reminder @!C.. Laboratory test due

If a request is sent to the district nurse to take blood,  a blood form should be completed and put in the district nurses tray. The clinical information on the form sent to the district nurse must include the current problem.

Shortcut labo or use macro.
Enter tests required as free text. Eg: FBC ESR U&E LFT TFT
Send patient to make appointment for blood test If this test must be done same day send note with patient, otherwise no paper required.
Phlebotomy Check reminder @!C.. Laboratory test due to see which tests required. R, highlight reminder then C
Answer reminder.  
Take blood.  
Complete form. Make sure the details include the patient's date of birth. The clinical details MUST begin with the current problem heading for the reminder. The name of the clinician requesting the blood tests should be entered on the request form unless stated otherwise.

Bloods taken according to secondary prevention protocols are assigned to the following nurses on the request form:

  • Diabetic annual review → Usual GP
  • Diabetic checks → Nurse 1 (Jane)
  • CHD NSF → Nurse 2 (Mandy)
  • Hypertension → Nurse 2 (Mandy)

 

Care must be taken that the text for the requesting clinician is added to the form. This may be a doctor or nurse1, nurse2 etc
  • Nurse1 = Jane Smithson
  • Nurse2 = Mandy Thompson
  • Nurse3 = Linda Crowe
  • Nurse4 = Gillian Kerr
  • Nurse5 = Judy Fairbairn
  • Nurse6 = Liz Martin
  • Nurse7 = Moira Henry
Results back.
  • Most results returned electronically using Lablinks
  • Paper results added to computer using appropriate battery codes. Paper results not sent by Lablinks are added manually.
Stephen stamps any paper results with action stamp.
Paper results to doctor. Dr ticks appropriate action and puts ALL results in computer update tray. Results do NOT ever go from Dr to receptionist for filing direct.
Paper results action added
  • Doctors on holiday: checking Dr NOT to tick boxes unless urgent action required. If urgent action is required, checking Dr is responsible for action and action stamp.
  • Telephone results added with "verbal report only" in free text.
  • No results to be filed unless initialled by Stephen.
Paper filing Filed by receptionist. Paper reports usually filed into holding box, kept 3 months then shredded.
