District nurse OOH referrals

Eligibility Criteria
- The patient has a health problem which requires skilled district nursing
intervention to ensure effective treatment and care in the home/primary care
setting.
- The patient has been given a diagnosis of terminal illness and requires
district nursing care to enable them to die at home where this is their wish and
the wish of the famly or their carer.
- The patient is already receiving care from other services but requires
district nursing assessment.
- Skilled nursing is needed to support patients/carers using technical
equipment to manage their health condition at home e.g. for administration of
drugs/feeds/respiratory equipment.
District Nursing referral priorities
To undertake care management, skilled clinical intervention and health education
in support of
- Teaching self care procedures to enable patients to manage their own health
care needs.
- Educating and assisting patients/carers to adapt to limitations imposed by
health related problems
- Preventing health complication associate with immobility/disability/existing
illness.
- Wound management
- Longer term management and support of patients with chronic degenerative
conditions
- Palliative Care
- Post operative follow up at home.
- GP prescribed treatments in the home
Examples of inappropriate referrals to the District Nursing Service
- Mobile patients who are able to attend the GP surgery or practice nurse clinic.
- Social/or check visits where no nursing intervention is required
- Patients only needing help with washing/bathing and for dressing and toileting
unless this is part of a wider care package/or in order to facilitate at
hospital appointments
- Diagnostic visit.
- Administration of oral medication.
- Emptying or changing urinary drainage bags.
- Routine skin treatments
